I'm not a lawyer and I haven't been in the wedding gig business for a long time but here's what's been working for me:

Wedding Music Agreement
This Agreement is made on , 2003 between Meerenai Shim (musician), of *my
address goes here*, and *clients names go here*

Meerenai Shim agrees to provide classical chamber music for the wedding
ceremony and reception of *names of couple goes here* (the
couple) on
*date of service goes here* .
Music for the wedding ceremony will start at 00:00 PM (time) at
*name/address of location goes here* (location). Music for
the wedding reception will start at 00:00 PM (time) at
*name/address of location goes here* (location) and end
at 00:00 PM (time).
The classical chamber music ensemble will consist of flute/violin/cello. This ensemble will perform appropriate standard classical repertoire and wedding music to be determined by Ms. Shim unless the client makes a specific request for other pieces to be performed. The client must make the request for specific music at least thirty (30) days before the wedding date.
Payment terms are 100% of the balance less the deposit amount of $00.00 payable on the day of the wedding. The hourly rate charged will be $00.00.
Each party agrees to give the other party a minimum of thirty (30) days notice prior to the wedding date should either party decide to cancel this agreement. Deposit is non-refundable if client cancels. If the musician cancels the deposit will be refunded in full. Client also agrees to limit musicians liability with respect to musicians own mishap or oversight to the total amount of money paid by the client to the musician.
